Set aside the usual ‘stability activation’ framework for thinking about abdominals – which dumps the rectus in the ‘almost evil’ muscle sin-bin. Instead, consider the abdominals and lumbo-pelvic region functionally. In all those years of focus on transverses Mark believes we have forgotten the functional importance of the rectus for lumbo-pelvic control. In running and walking, the rectus works to hold up the pelvis against gravity, preventing a collapse into anterior tilt and lumbar lordosis. It does the same job in water, holding the swimmer up in good body position, preventing a loss of streamlining and drop in speed. In all these situations, the rectus contracts isometrically – ie, without any change in muscle length. We need to assess and exercise isometrically in a functional position. So you can see how crunches or sit-ups are not going to be useful, either to test isometric endurance, or to strengthen the muscle.

After injury, the good therapist aims to rehab their client to maximise both future performance and injury prevention. When it comes to restoring balance, it is not sufficient to think purely about improving a client’s proprioception (their joint position sense and detection of movement). We need also to restore normal neuromuscular coordination. And this is done best when the exercises are directly relevant to the functional and dynamic positions of the client’s activity or sport.

Mark Alexander explains how to manage a nasty ankle sprain immediately after it happens. The regime outlined has been used in elite sport for years but hasn’t entered into mainstream injury management protocols. The reason for this is simple: it is extremely uncomfortable! But, as Mark explains, it really does work:
“I have seen athletes on crutches after sustaining diagnosed Grade 2 2+ ankle sprains who were able to walk without crutches with only a minimal limp after their first session of this therapy, and who were back training after three to four days (obviously with a lot of tape support). SIB readers will be familiar with what happens after an ankle sprain: internal bleeding, inflammatory processes, pain and swelling. The brain also gets involved, producing muscle inhibition and a reduction in proprioception, which usually forces the injured athlete to limp in an effort to reduce pain. By numbing the ankle and tricking the brain into allowing the ankle to move through a normal range of movement without pain, I believe we can minimise the detrimental effects of ankle sprains.” Mark Alexander
